Chronic Obstructive Pulmonary Disease (Copd) Case Management.This paper presents a nursing case management plan for an elderly (85 year-old) female patient with Chronic Obstructive Pulmonary Disease (COPD). Case assessment is made through application of the Betty Neuman Systems Model, including identification of client stressors, lines of resistance, lines of defense, and variables. Following assessment and description of the need for a case manager as well as an overview of the interdisciplinary team and community services appropriate to the client, plans for communication among all relevant parties, provision of continuity of care, monitoring, and evaluation with appropriate alternative interventions are presented.
