Drawing on the Betty Neuman Systems Model for case assessment, this paper provides a nursing case management plan for an 85-year-old female with multiple chronic illnesses including congestive heart failure, chronic renal failure, and type 2 diabetes. Following assessment including identification of client stressors, lines of resistance, lines of defense, and variables, the paper describes the need for a case manager with the client and the appropriate interdisciplinary team and community resources. A plan for communicating and coordinating care along with a plan for monitoring and ongoing evaluation of the patient’s progress, along with appropriate alternative interventions is presented.
